Refractive Lens Exchange
A Refractive Lens Exchange is a vision correction procedure where the patient's natural lens is expertly removed and replaced with a premium intraocular lens to reduce and in most cases eliminate the need for glasses.
The surgery is exactly the same procedure that is used in cataract surgery. Even though the patient does not have a cataract, the natural lens is removed and replaced with a premium artificial lens. Various types of premium intraocular lenses can be chosen based on the individual needs of the patient. Dr Lourens will go through all the various options of the different premium lenses during the initial vision correction consult. As Dr Lourens is a key opinion leader of all the major lens manufacturing companies, each patient will have access to all the various lens options and latest technology available in the world.
An extremely comprehensive eye examination will be performed to assess the suitability of each individual’s eyes for lens replacement surgery and the implantation of a specific premium intraocular lens.
Who is suitable for Refractive Lens Exchange?
Any patient who is older than 45 years of age who are dependent on glasses is suitable for a refractive lens exchange. At Crystal Eye and Laser however, we prefer to perform Presbyond if you are suitable for laser eye surgery. However, not everyone is suitable for laser eye surgery and then these cases a refractive lens exchange is preferable.
Suitable candidates for lens replacement surgeries are:
- 45 years and older
- wearing multifocal or reading glasses
- wearing contact lenses to see clearly
- wearing mono-vision contact lens prescriptions where one eye sees in the distance and the other for near
- patients with high prescriptions not suitable for
Presbyond
- any patient wishing to be less reliant on glasses
Intraocular Lens Options
Monofocal
These lenses provide great vision in the distance. Glasses for intermediate and near viewing will be required
EDOF
These lenses use advanced optics to elongate your focus. Glasses may still be required for some of the time such as reading fine print however there are fewer side effects. EDOF lenses are often known as a “social” lens which means you can do most of the things you love without the need for glasses.
Multifocal
The trifocal lens design will allow 3 focal points and you will see clearly in the distance, middle and near. There may be minor effects like haloes at night but most people adapt over the first 3 months after surgery.
Monofocal Plus
These lenses are an excellent option for patients that have a very active lifestyle and who do not mind wearing reading glasses. Monofocal PLUS lenses uses natural optical phenomena known as spherical aberrations to enhance the overall depth of field and give patients a good range of vision from about arms length to the far distance. These lenses are especially excellent in patients who are used to monovision contact lenses as 1 eye will be aimed at near and the other eye for distance to give the patient a full range of vision all round. These lenses do not cause any glare and halos and is safe to drive with at night time. They are also especially useful in golf and tennis players who requires excellent distance and intermediate vision.
REFRACTIVE LENS EXCHANGE PROCEDURE
The refractive lens exchange surgery is performed as a day procedure.
The surgery is performed in a hospital facility all day surgery.
A refractive lens exchange takes approximately 10 minutes per eye.
The surgery is performed under intravenous sedation and local anaesthetic. We also accommodate patient's want to surgery performed under general anaesthesia.
Absolutely no injection is performed in the orbit to numb the eye by Dr Lourens.
As Dr Lourens has advanced training in lens replacement surgeries the procedure is performed under drop local anaesthesia.
The procedure itself is performed through a 2 mm incision which is self-sealing and no suturing is required.
The natural lens is expertly removed and replaced with a premium lens.
As per standard both eyes are operated on the same day that we also accommodate patients who only wants 1 eye operated at a time.
The recovery following lens replacement surgery is usually 2 to 3 days

Dr Lourens van Zyl

Cataract & Laser Refractive Surgery
Dr Lourens is the principal opthalmologist at Crystal Eye & Laser Centre. He is a fellow of the Royal Australian and New Zealand College of Ophthalmologists and completed his Ophthalmology training in Adelaide, South Australia. He holds fellowships from the Royal College of Surgeons in Ireland, The Colleges of Medicine of South Africa and The European Board of Ophthalmology.
He has special interest in Laser Eye Surgery and advanced Vision Correction surgery including cataracts. He has post graduate fellowships in Advanced Corneal and Anterior segment surgery and Cataract and Refractive Surgery. Following his post graduate fellowships Dr Lourens completed a 1 year post graduate diploma followed by a 3-year Master’s degree in Cataract and Laser Eye Surgery through the University of Sydney.
